Formstack Sign: Some Documents finalized without audit pages
Resolved - We identified an issue that caused some Formstack Sign documents to be finalized and delivered without an audit trail.

The issue affected documents on October 8 from 7:42 PM to 9:31 PM ET.

The signing process has recovered, and newly completed documents are processing normally. We are implementing a recovery script to restore the missing audit trails for affected documents.

As part of this recovery process, some customers may receive a duplicate notification that their document has been finalized. If you previously received this notification, no action is required, and the duplicate message can be disregarded.

Sign Integration Authentication Issues
Resolved - The issue causing some Formstack Sign connections to lose authentication unexpectedly has been resolved. A fix has been deployed, and authentication has been restored for affected connections.

Customers should now be able to process Documents-to-Sign deliveries normally. If you continue to receive an authentication error, please reauthenticate your Formstack Sign connection and retry the delivery.

Monitoring - A fix has been deployed to prevent Formstack Sign connections from losing authentication unexpectedly, and we are monitoring the results.

We are also working to automatically restore authentication for previously affected connections. Customers who need to resume processing immediately may manually reauthenticate their Formstack Sign connection. We will provide another update once the restoration process is complete.

Identified - We have identified an issue causing some Formstack Sign integrations, including Documents-to-Sign deliveries, to lose authentication unexpectedly. Affected deliveries may fail with an authentication error, even if the connection was recently reauthenticated.

Our Engineering team is actively working to deploy a permanent fix. We will provide another update as soon as the fix has been released.
